Medical Release, Assumption of Risk & Liability Waiver

Legacy Volleyball – 2026-27 Season

1. Programs & Activities Covered

This Agreement applies to all Legacy Volleyball programs and activities, including but not limited to private lessons, travel teams, academies, camps, clinics, open gyms, strength and conditioning sessions, performance combines, tournaments, scrimmages, training sessions, fundraising events, transportation activities, and any related event conducted now or in the future.

2. Assumption of Risk

I acknowledge that volleyball and athletic training involve inherent and significant risks of injury, illness, disability, or death. Risks specific to volleyball include, but are not limited to: diving and floor contact, jumping and landing injuries, collisions at the net, sprains, fractures, concussions and other head injuries, heat illness, muscle and ligament damage, overuse injuries, equipment failure, the negligent acts of other participants, and travel-related risks. I knowingly and voluntarily assume all such risks, whether known or unknown.

3. Strength, Conditioning & Performance Training

Legacy Volleyball may conduct athletic performance training including vertical jump testing, sprint testing, agility drills, plyometric exercises, resistance training, core strengthening, weighted training, speed development, and performance tracking. These activities increase physical demands and risk of injury. I voluntarily consent to my athlete's participation and confirm my athlete is physically able to participate.

4. Medical Authorization & Financial Responsibility

I certify that my athlete is physically fit to participate. In the event of injury or medical emergency, I authorize Legacy Volleyball representatives to obtain medical treatment and share relevant medical information with providers. I accept full financial responsibility for any medical expenses incurred. Legacy Volleyball does not provide health or accident insurance.

5. Concussion Acknowledgment

I understand the risks associated with concussions and agree that my athlete will report symptoms immediately and will not return to play until cleared under Legacy Volleyball's Concussion Protocol & Return-to-Play Agreement, which is incorporated into this Agreement by reference. Legacy Volleyball recommends, though does not require, annual ImPACT baseline concussion testing, available through local sports medicine and concussion clinics.

6. Facility & Multi-Location Coverage

Legacy Volleyball conducts activities at multiple facilities, including but not limited to Global Academy (formerly Star Center), 9768 Warren H Abernathy Highway, Spartanburg, SC 29301; Riverside Baptist Church Gym; area schools; tournament venues; rented facilities; and other locations used presently or in the future. I understand conditions may vary by facility. This waiver applies to all current and future locations. I release and hold harmless both Legacy Volleyball and facility owners/operators.

7. Equipment Responsibility

Participants may use volleyballs, ball carts, nets, antennae, jump boxes, weights, resistance bands, agility ladders, cones, blocking pads, and other training equipment. I agree that equipment will be used properly and under supervision. I accept financial responsibility for intentional or reckless damage. Legacy Volleyball is not responsible for lost or stolen personal items.

8. SafeSport & Athlete Protection

Legacy Volleyball adheres to athlete protection standards aligned with USA Volleyball and the U.S. Center for SafeSport's Minor Athlete Abuse Prevention Policies (MAAPP), including our Emergency & Coach Transportation Policy provided separately. I understand misconduct concerns should be reported immediately, and that violations of conduct standards may result in disciplinary action or removal.

9. Payment and Cancellation for Private Lessons

I understand that when I book a private lesson a hold is placed on my payment method, that the final price depends on how many players train in the session, and that my payment method is charged automatically when the session ends. I may cancel free of charge up to 24 hours before a session. Cancellations inside 24 hours, and no-shows, are charged in full. If Legacy or the coach cancels, nothing is charged.

10. Release & Indemnification

In consideration for participation, I release, waive, and discharge Legacy Volleyball, its directors, officers, board members, coaches, volunteers, staff, sponsors, affiliated organizations, and facility partners from any and all liability arising from participation. I agree to indemnify and hold harmless Legacy Volleyball from any claims, damages, losses, or expenses arising from my athlete's participation.

11. Governing Law & Severability

This Agreement shall be governed by the laws of the State of South Carolina. If any portion of this Agreement is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.
